Correlation of Her-2 immunohistochemistry with FISH result and comparison with other laboratories. HMC Qatar

While there is no recommended percentage for positivity in her-2 FISH, a lot of times the clinicians complain that her-2 FISH was ordered in too many cases and the result took 2 weeks and delayed treatment. This audit showed that from Jan to June 2021, Her-2 FISH was ordered on 107 cases out of which almost 16% of the her-2 FISH were positive (amplified), close to the ASCO reported 23% and within the 15-35% range reported in other papers, concluding that while steps like second opinion in cases of her-2 2+ score might be helpful, the frequency of ordering her-2 FISH wasn’t too high.

To find out if its once in a while that ordering physicians miss filling some of the fields or it’s a wider problem. Audit showed that only 33 % (87/258) of the forms were fully filled.

The findings were discussed in the department, and we agreed that doing a grand round on this topic will increase awareness and introducing electronic request forms will decrease the issue. A lot of sentinel lymph nodes lacked the radioactive sticker. This was discussed in detail with the radiation expert who reassured that the lab staff was not in danger.

Re-Audit of cervical pathology, Royal Liverpool hospital.

I conducted the re-audit of cervical pathology specimen (biopsy and LLETZ) as part of the national cervical screening programme. The last audit was done in 2022 that showed a higher rate of CIN1 (84%) and lower rate of benign (7%). The re-audit showed 19% benign and 80% CIN or above. This was closer to the national average. The turnaround time for the biopsies and LLETZ was lower than the target but similar to the national average.

Audit of endometrial cancer resection cases reported in 2025 showed 100% concordance with Rcpath dataset. More than 90% of the cases were reported in less than 30 days.
